Your First Visit

Thank you for choosing Dr. Stewart. Here is everything you need to know to prepare for your first appointment at Chippewa Valley Orthopedics and Sports Medicine.

Your Appointment, Step by Step

We want your first visit to be as smooth and comfortable as possible. Here is what typically happens from the moment you arrive.

Check In & Paperwork
Our front desk will greet you, take a copy of your insurance cards, update your patient demographics, and assist you in completing your new patient forms. To save time, you can download and complete the forms below before your appointment and bring them with you.

Meet with the Physician Assistant
You will be brought to one of our examination rooms where Dr. Stewart's physician assistant will carefully listen to your symptoms, gather your complete medical history, and perform an initial assessment. We will often ask you to get onto the examination table so we can thoroughly evaluate the involved joint or joints.

Imaging
An x-ray is often necessary to evaluate your condition and determine a specific diagnosis. We are fortunate to have on-site imaging available the same day as your appointment, so Dr. Stewart will have everything he needs when he comes in to see you.

Consultation with Dr. Stewart
Dr. Stewart will examine you personally, carefully explain your diagnosis, walk you through any abnormalities on your imaging studies, and lay out the full range of treatment options available. He takes time to listen and answer your questions thoroughly — you will leave with a clear understanding of your condition and a plan moving forward.

Next Steps
Depending on your diagnosis, next steps may include scheduling additional imaging such as an MRI, beginning physical therapy, arranging for an injection, or scheduling a surgical procedure. Our team will coordinate all follow-up and ensure you understand each next step before you leave.

What to Bring

Bringing the following items will help us provide the most accurate and complete care from the very first visit.

  • A valid photo ID and insurance card(s)
  • A complete list of all current medications, including dosages
  • Any relevant prior imaging (x-rays, MRI, CT scans) on disc or digital format
  • A list of prior surgeries and dates, if applicable
  • Completed new patient forms (see below — print and bring, or complete at arrival)
  • A referral from your primary care physician, if required by your insurance plan
  • A trusted family member or friend if you would like support during your consultation

Arrive 15 minutes early if you have not completed your new patient paperwork in advance. This gives our team enough time to check you in and enter your information before your scheduled appointment time.

Please review and complete the registration forms below before your first appointment. Returning completed paperwork saves time and helps our team serve you more efficiently. You are also welcome to complete the forms upon arrival.
